Female Student Of Tarkwa SHS Swore An Affidavit To Demonstrate Her Ownership Of Two Names.

An affidavit has been made by a female student of Tarkwa Senior High School to demonstrate her ownership of two names.

She acknowledged that her name appears under the names Janet Damoah and/or Janet Esi Damoah in some documents and records.

She maintains that all of the records and documents that identified Janet Damoah and/or Janet ESI Damoiah still apply to her and are legitimate.

She continued by saying that she hoped that the relevant authorities would be made aware of her declaration, which was intended to absolutely declare and affirm that her name is Janet Esi Damoah.

Janet Esi Damoah, a citizen of the Republic of Ghana who resides in Prestea in the western region, genuinely and seriously declares the following on this third day of January, 2024:

” I, Janet Esi Damoah, a resident of Prestea in the western region of the Republic of Ghana, sincerely and solemnly declares as follows”:

i. That, I am the Declarant herein and a Ghanaian by birth and Nationality

ii.  That the facts deposed to are personally within my knowledge.

iii. That, some of my documents and records show my name as Janet Damoah  and or Janet Esi Damoah.

iv. All the documents and records that Captured JANET DAMOAH and or JANET ESI DAMOAH all refers to me and still hold good and valid.

v. That I by this declaration wish to state and confirm categorically that my true and correct name is JANET ESI DAMOAH and pray the authorities concerned to be informed accordingly.

Wherefore, I make this solemn declaration in support of my above statement.
